I submit this Vaccination and Microchip Voucher Application Form with the relevant supporting documentation as required. I declare that the details are correct to the best of my knowledge. I am aware that, I, the applicant, must be the responsible person for the animal.

Privacy Notice

Council deals with personal information in accordance with law including the Information Privacy Act 2009. Details provided in this form will be given to participating vets for the purpose of verifying the voucher for the vaccination and microchipping services. Council has partnered with Paws for a Purpose to deliver this program, and will provide microchip number, dog details and street to this group for carrying out research into Canine Parvovirus.

This form is to be completed when applying for a vaccination voucher or vaccination and microchip voucher for a dog/s. Vouchers will be supplied to the participating veterinary surgery and will cover the total cost of the vaccination and/or microchip procedure. Vouchers expire on 30 April 2022 and are applicable to Rockhampton Region residents only.
